How to request Blue Cross Blue Shield ID Cards?

If you are a current client of Smart Insurance Agents LLC, we can request your ID cards and have it mailed to you. If you are NOT a current client of Smart Insurance Agents LLC, you would need to either contact your health insurance agent, or call Blue Cross Blue Shield Direclty. 

To request ID Cards from us: 

1. Got to https://www.getsmartquotes.com/contact-us/
2. Fill in the form, make sure you enter your full name correctly.
3. In the "Message" box please write: I need a set of my BCBS-IL ID cards to be mailed to me.
